Get your Testing Area Survey here! Please return the completed form for your $1 credit! Testing Area prices are the same as regular 6ml bottles ($5.00), but a fully completed survey will earn you a $1 store credit available on a future purchase. You will only be credited for one survey per flavor, so make it count! Don't forget to include your name and order number on the survey to be able to receive your credit. Surveys must be received by Ms T's within 10 DAYS of the SHIPPING DATE of your order to receive the $1 credit. We need this info as soon as possible to make timely changes to the flavors.

This free item will attach to your order, and you will be provided with a "download files" link in your confirmation email. It will be next to the "Testing Area Survey" item in the list of items your ordered. If you forget to pick one up with your order, you can always place an order JUST for the survey, and you will not be charged anything. The files are available for download immediately after you receive your order confirmation email.

When you go to download the file, pick which file format is easiest for you to use and fill out. You can download all three to see which works best for you if you like.  The three formats you will see after clicking the "download files" link are:

Acrobat (.pdf)
Word (.doc)
Plain text (.txt)

This feedback is important to us because it allows us to get the flavor right before we put it on the site for regular purchase in all sizes. The fewer surveys we get, the longer the product may remain in the Testing Area. We want your actual thoughts on the juice, good OR bad. 

Remember, you can order the Testing Area flavors without supplying feedback, but returning surveys will earn you your credit. Credits will be tracked, so only one credit will be given per flavor, even if you provide multiple responses. Thanks for your feedback!!
